Forms

Upon scheduling your first session, I provide login information to a portal where you’ll find the following paperwork to complete online prior to meeting:      

  • Disclosure statement
  • Privacy policies
  • Intake form  

Payment

I accept cash, check, or credit at time of service. If you choose to store your card information in the secure client portal, services will be charged at the end of each session.

I am in-network with Cigna/Evernorth and am otherwise considered an out-of-network provider. Your insurance benefits may allow reimbursement of all or part of your payment. If I am out of  network for your plan, I am happy to provide you with the necessary paperwork to file for reimbursement from your insurance company or employee benefit plan.
